forms P85 and P86

Hi,

I know, that last year form P85(S) has been replaced by P85 and form P86 has been discountinued. So am I correct, that if a person would like to apply for a tax refund (and they haven't filled the form P86 when arrived to the UK) all they need to do is send form P85 along with P45 ? Can it be a copy of the P45 or the last payslip if the original document is lost ?
