Got one this afternoon from a client and noticed the Doctor expects the baby to be born in the week that contains 15/01/13. The form was signed 11/09/13.
So, it is obviously an error. Do I have to go to the bother of getting her to get a new form or will this be acceptable with the mistake included?
Who is the form actually for? If it is for HMRC then something will need to be done (which I will need to find out) or is it for the Employer - in which case I think they will accept it with the error included. It isn't an anonymous employee in a huge company it is the only employee other than the owner.

This form is for the employer you don't have to send it to HMRC. Personally though I would ask for this to be changed, even though it is obviously a mistake. Don't want anything coming back to bite you on the backside later!
Hi Peasie I agree with Pauline although I recall something on the HMRC website about alternative medical evidence as long as it shows when the baby is due, so perhaps a letter from the doctors will do if you are struggling getting another form?
