Fixed Asset Register Template

I have a new client with lots of company assets and I want to send him a Fixed Asset Register form to complete and send back to me.  Does anyone have a template they use that I could borrow please?

Two options
a) Just set up a spreadsheet with Columns; Asset number (Barcode), Type, Make, Model, Serial No, Location, Purchase date, Cost. This should take care of most clients needs and each can add more details when visiting every asset worth more than say £50 such as condition, PAT test result.
b) The paper based solution is very time consuming for any client with more than a few hundred items, and you could add much more value to your client services on reviewing www.assettrac.co.uk where paper free solutions are much more efficient. Call me if needed.
