Pension lump sum withdrawal. What goes where on tax return?

Hi all, I have a question. I have a client who withdrew all his private pension with Scottish Widows in which I have the P45. The payment he received into bank was £19082. Tax free amount of £6614, taxable amount of £19844, tax deducted £7377. When it comes to his tax return do I fill in the UK pensions annuities and other state benefits, Pensions (other than State Pension), retirement annuities and taxable triviality payments part on Part 4 of the return? I'm just a little worried that it's not the correct part as it doesn't ask for the PAYE ref number of Scottish Widows xxx
