Have you sent a professional whatsit doodah letter? Sorry, forgotten what they are called!!!! Has your client given him authority to speak with you? If not he won't. Also, is your client up to date with his bills from the old accountant?

Yey, I still didn't have it until you said it. Not sure if my memory is getting worse, or just on overload with work!
I would try ringing as people are usually less rude to your 'face' so to speak. If that fails, perhaps get your client to ring. Otherwise you might have to threaten reporting him to his professional body, but I would suggest that is a very much last resort thing. Quite often I've found it's usually just sparing the time to pull everything together to pass on ie the I'm not being paid to put someone on this task so will do bits when they have spare time and if they are anything like use, they have no spare time! Does of course also depend on how long your list is!

You could always ask your new client to get it for you. Most accountants will provide the ex-client information if they ask, simply because they don't want to be rude for the sake of it.
