Assuming it's not changed since my version, there should already be a suitable statement that you can customise to suit your needs.
Go to the customers screen, and before doing anything else, pick a suitable customer for test purposes (this can save time if you happen to have a *lot* of customers!) - click on that customer to highlight it.
Now click on the 'Statements' icon along the top. This should open the 'Report Browser' - click on 'Layouts' in the left hand column. This is where all the pre-designed statements can be found, with meaningful-ish descriptions in the 'Name' and/or 'Description' columns.
Scroll down the list and look for the ones that might closest match your requirement. I'd assume you want A4, for example, so look for ones with that in the name/description, and ones that say something like "with tear off remit" (based on what mine says). There will probably be a few.
Move your mouse over one that sounds like it might be suitable, and click on preview to see what it looks like and decide if it's ideal as a starting point, then close it. If it wasn't ideal, move onto another, and another.
When you find one that's suitable, click on the star icon to the left to mark it as a favourite - it'll now be listed when 'Favourites' is clicked on the left, so should be easier to find.
Move the mouse over it again, and right-click to call up a menu, and click 'Edit' in that menu - this will launch the report in the report designer.
Near the top there should be a tool bar with a number of things on it starting with "Pointer" then "Add Text", and so on - on that line you should see one "Add Image/Logo". If you click on that, and then somewhere on the report, a dialogue box will appear to allow you to select a graphic file on the computer and include it on the report - so that's what you need to do.
Depending on the layout you've chosen, and the nature (size etc) of the logo, you might have to move things around - but you should be able to work out how to do that just by playing with it.
